The brand
While the brand was initially created and managed by Halton Healthcare, it was eventually handed off to representatives of the CCHOHT for management and oversight, transitioning from Halton Healthcare to ensure brand consistency.
The Connected Care Halton Ontario Health Team (CCHOHT) is a partnership of health care providers in the Halton Hills, Milton and Oakville communities. The Ontario Health Team model is part of the Ontario Ministry of Health's health system transformation strategy.
